Your treatment begins with a one-on-one consultation with Alexandra Pales, ARNP, PMHNP-BC. This includes:

  1. A clinical interview to understand your history and symptoms
  2. Screening tools to assess the severity of depression
  3. A discussion of treatment goals and preferences

Step 2: Personalized Treatment Plan

We believe in collaborative care—you will be actively involved in your treatment decisions. Your plan may include:

  • Patient-specific supplement recommendations

  • Bloodwork evaluations to rule out nutrient deficiencies or medical contributors

  • Medication options, including SSRIs, SNRIs, or others tailored to your concerns

  • Psychiatric evaluations for related conditions like depression, OCD, or PTSD

  • Referral to specialized therapy, including CBT, DBT, or EMDR

Step 3: Ongoing Support & Adjustments

  • Follow-ups are scheduled every 2-4 weeks to monitor progress.
  • Once stability is achieved, appointments are spaced every 8-12 weeks for continued support.
  • Medication adjustments or therapy recommendations can be made as needed.

Our initial consultation is 60 minutes and includes a full mental health evaluation.

How long are follow-up appointments?

Follow-ups are 30 minutes. These sessions include medication management, lifestyle review, and brief supportive therapy.

What if I’m not ready to start medication?

That’s completely okay. There is no pressure to take medication. We offer education, explore all available options, and go at a pace that feels right for you.

What if I’m worried about side effects like sleepiness or weight changes?

We’ll discuss your goals and concerns in detail. If you want to avoid specific side effects, we’ll explore medications that minimize those risks—or recommend alternatives.

What if I’m in crisis or experiencing panic attacks?

Where do appointments take place?

All care is delivered virtually through a secure, private telehealth platform, accessible via computer, tablet, or phone. You’ll need a quiet, private space and working video/audio for the session.
